AFC Social Worker

AFC Social Worker

The AFC Social Worker is part of a health care team that provides social work and case management for the Adult Family Care (AFC) program. Responsibilities include assessment and evaluation of potential program participants and caregivers, care plan development, care coordination and monitoring, crisis intervention, education of host caregivers and AFC participants, and collaboration with AFC nurses. Manage an AFC caseload by providing ongoing clinical casework/case management to program participants, in compliance with program policy and procedures, and Medicaid guidelines. Provide psychosocial support to program participants and caregivers; addressing issues related to caregiver stress, family systems, mental health diagnoses, and aging and disability. Maintain accurate and timely case records, including all required forms, case documentation, and statistics, according to agency, program and professional standards.

Requirements: Bachelor’s Degree with at least 2 years of recent clinical experience working with elders or individuals with disabilities is minimum requirement. Possession of, or eligibility for, Social Work licensure required. MSW preferred but not required. Knowledge of the home care system and long term care issues required. Excellent organizational and written and oral communication skills required. Ability to work collaboratively as part of a team essential. Knowledge of human development, including the areas of physiology of aging, mental health, and substance abuse essential. Experience using Word and strong data entry and work-processing skills essential. Bilingual in Spanish, Portuguese, Haitian Creole, or other language preferred. Must be willing and able to use own vehicle.

Compensation: Low-Mid $40s and an excellent benefits package which includes medical, dental, short & long term disability, life insurance, 401(k) with agency contribution, flexible spending account, employee assistance program, 24 days of accrued earned leave, a 35-hour work week and 13 paid holidays.
